We usually ask, "Do you have the time?" in person. For example, you forgot your watch and you walk up to a stranger and ask, "Do you have the time?" It is a very polite way of asking. So I have only heard this when asking strangers and in this situation.
"What time is it now?" can be asked in person or over the phone or texting, etc.
Yes, both expressions mean the same thing, but the first expression is only used in a specific situation. The other expression can be used in any situation. Can you understand what I mean?? :)
